Panel 1 [Adelaide speaks to Barzillai as he shrugs off his jacket. He squints at her, looking baffled at her suggestion.]
ADELAIDE:
Besides, Mr. Roxbury didn’t pay for this ship to have a doctor. I might very well be stepping into the role.
BARZILLAI:
As doctor?
Panel 2 [Adelaide looks pointed despite her nausea, determined to not be underestimated.]
ADELAIDE:
I’ve done it before. I cared for you, didn’t I? When you were the only one who came back.
Panel 3 [Barzillai rubs the back of his neck]
BARZILLAI:
…I suppose you have as much experience as any of us, for whatever it’s worth.
Panel 4 [Barzillai takes her hand, head slightly lowered, giving her a wary look. Shot over Adelaide’s shoulder.]
BARZILLAI:
But I ask that you don’t repeat that part of my history to any of the others.
Panel 5 [Barzillai loosens his cravat, frowning.]
BARZILLAI:
Some might think it bad luck, and I don’t want to color the voyage in such a way.

Is that Brutality with the little “Shh” on the captain’s neck?

I’ll let that interpretation be up to you! Is it just a sound effect of fabric on fabric, or a ghostly voice? Either works!
